Admission Requirements Submission

To all enrolled college freshmen and transferees, kindly submit your complete admission requirements in hard copies to the HAU Admissions Office on or before September 10, 2020.

The list of the said requirements are as follows:

For Freshmen:

  • Original and photocopy of Form 138 (Report Card)
  • NSO/PSA Birth Certificate (photocopy)
  • Marriage Certificate (if student is married)
  • Recommendation letter from Senior High School
  • Undertaking signed by student and parent
  • One (1) copy of 2x2 latest photo

 

For Transferees:

  • Honorable Dismissal
  • Certified True Copy of Grades
  • Certificate of Good Moral Character
  • NSO/PSA Birth Certificate (photocopy)
  • Marriage Certificate (if student is married)
  • Barangay Clearance
  • One (1) copy of 2x2 latest photo

 

For those who have already submitted but are incomplete, kindly submit the rest of the needed requirements.
