College Enrollment Procedures

College Freshmen

Steps:

1. Present Form 138 at the Admissions Office
2. Pay processing fee at the Finance Office
3. Get Admission Kit at the Admissions Office
4. Fill out HAU-ADM Form #1 (CET Application Form)
5. Get CET schedule and take the CET
6. Have HAU-ADM Form #3 (Recommendation Form) filled out by HS Guidance Counselor or by the class adviser
7. Go to the University Guidance Center for test result and individual interview with Guidance Counselor
8. Submit the following documents in HAU yellow envelope at the Admissions Office:
• College Application Form
• Original Form 138
• Birth/Baptismal Certificate (photocopy)
• Marriage Certificate (if married)
• Undertaking
9. Get Permit to Enroll
10. Proceed To Enrollment

Transferees

Steps:

1. Present the following credentials to the Admissions Officer:
• Transfer Credentials
• Certified True Copy of Grades for evaluation purposes
2. Initial interview by the Admissions Officer, Dean (for Accountancy, CICT, CASED, Nursing students only)
3. Pay processing fee at the Finance Office
4. Get Admission Kit at the Admissions Office
5. Fill out CET Application Form
6. Get CET schedule and take CET
7. Interview with Guidance Counselor (College Guidance Center)
8. Interview with Student Conduct Officer
9. Submit the following documents at the Admissions Office:
• College Application Form
• Transfer Credentials
• Certified True Copy of Grades
• Certificate of Good Moral Character
• Barangay Clearance
• Birth/Baptismal Certificate
• Marriage Certificate (if married)
• Undertaking
10. Get Permit to Enroll
11. Proceed To Registrars Office
12. Proceed To Enrollment
